Facility must exactly match a listed facility (or "Unassigned"); Unit, if given, must exactly match a unit in that facility.

CSV columns: First Name, Last Name, Status, Role, Assigned Facilities, Email, Phone. Email is required and doubles as their login username. There is no shared or default password: a new account cannot sign in until an admin gives it one with Reset, and whoever receives it must replace it before they can do anything.
